U.S. Cuts Vaccines Recommended for All Children from 18 to 11

A new executive order signed by U.S. President Donald Trump reorganizes childhood and adolescent vaccine recommendations into three categories. Vaccinations recommended for all children will cover 11 diseases, while several others will be advised for higher-risk groups or through shared clinical decision-making. The order also calls for further review of vaccine timing and sequencing and aims to expand parental choice.
The executive order, signed on August 10, 2026, introduces a three-tier framework for U.S. childhood vaccine recommendations. The first category includes immunizations recommended for all children: measles, mumps, rubella, diphtheria, tetanus, pertussis, polio, Haemophilus influenzae type B, pneumococcal disease, human papillomavirus and varicella.
The second category covers immunizations recommended for certain high-risk groups or populations. These include respiratory syncytial virus monoclonal antibodies and vaccines against hepatitis A, hepatitis B, meningococcal B, meningococcal ACWY and dengue.
A third category consists of vaccines recommended through shared clinical decision-making. It includes hepatitis A, hepatitis B, rotavirus, meningococcal disease, influenza and COVID-19.
According to the White House fact sheet, the number of diseases for which immunization is recommended for all children is reduced from 18 under the CDC’s 2024 schedule to 11. The remaining vaccines do not become unavailable; instead, they are placed in different recommendation categories.
The order also addresses the combined MMR vaccine. Once separate products are available domestically, it recommends administering measles, mumps and rubella vaccines as three individual shots. It also states that, where feasible, childhood immunizations should be given at separate medical visits.
Within 90 days, the HHS Task Force on Safer Childhood Vaccines must present plans covering several areas, including the timing and sequencing of childhood vaccines, continuous evaluation of vaccine risk-benefit profiles, improved vaccine safety monitoring, greater transparency and additional research.
The order also advises U.S. states and territories to review relevant laws and regulations governing immunization requirements, including those connected to school enrollment and attendance.
